Billing
Fees without Insurance:
Sessions are 45 to 90 minutes. Fees start at $20 and range to $250,
depending upon the type of service, the counselor, and their level of
experience and/or education. Sessions extending beyond 45 minutes
usually cost more. Please discuss the specifics with your counselor.
Fees with Insurance:
The fees we charge are billed directly to your insurance company.
Payments of these fees are usually contract amounts preset by your
insurance company. These fees are based upon the insurance company’s
formula for “allowable” or “usual and customary” fees for specific
“coded” services and are generally unknown to us. These fees and your
benefits can change and insurance companies do not guarantee policy
benefits or payments. Once benefits and co-payments are established,
your responsibility will be only whatever your normal co-payment is
unless your insurance company refuses payment. Please be prepared to pay
$100 towards your first visit pending confirmation of your exact
benefits in writing; any overage will be applied only to the part you
are responsible for; unused amounts will be returned to you. Please
note: you are personally responsible for any sessions unpaid by your
insurance company; this includes missed, no show, or late canceled
appointments which are billed directly to you at the regular amount.
Sliding Scale:
Hardship adjustments are limited and based on a gross household income
of $20,000 or less, plus $1/$1,000 of income in excess of $20,000.

Credit, Insurance,
Billing and Collection Policy:
Many insurance contracts cover a portion of outpatient mental health
sessions and usually have some maximum number of visits or financial
benefits. It is important that you know the details of your coverage
because you are responsible to see that your account is paid in full and
that you obtain whatever pre-authorizations and continuing
authorizations are needed. By signing this agreement, you are agreeing
that this document is the primary agreement between your counselor and
you, and Heart To Heart Counseling. The contract you have with your
insurance company gives you certain rights; those rights do not extend
to Heart to Heart or our counselors. While we might advocate on your
behalf from time to time, we cannot make the insurance company enforce
their agreement with you, only you have that right. This is the reason
that it is your responsibility to see that your debt for services here
is paid in full. Heart to Heart Counseling will not negotiate the
collection of insurance benefits. Unless otherwise agreed in writing all
fees and co-payments are due at the time of service, so no statements
are sent. Client with unpaid balances will automatically incur a $5
statement fee monthly. Unpaid fees and co-payments will incur a $5.00
processing fee per session. It is agreed between the parties that:
balances over 30 days will be assessed 18% interest and past due
balances will automatically incur $5.00 statement fees; Charges,
interest, and fees are subject to change without notice; In the event
collection or attorney services are contracted to collect any unpaid
account balance, client agrees to pay a late fee equal to the unpaid
account balance which will be added to the account balance at the time
of referral to the attorney or collection agency in addition to all
attorney fees with interest, plus those fees and such other relief
allowed by law and waives any right to a change of venue from Spokane
county.
Late
Cancellations, Missed No-show Appointments:
Uncanceled, missed appointments and those appointments canceled with
less than 24 hours notice are charged the full fee without regard for a
sliding fee, scholarship, or contract rate. The client is solely
responsible for canceled, missed or late cancel appointments; insurance
companies will not be billed for uncanceled, missed or late canceled
appointments.
